• Provide or assign resident care according to the needs of the residents under the direction of the supervisor and/or director of nursing.
• Maintain a home‑like, person‑centered care environment for all residents.
• Serve as a liaison between Home Health and Americare communities to provide appropriate care to residents.
• Give clinical guidance to nursing staff (PCA, C.N.A., CMT, etc.).
• Make rounds to assess resident and staffing needs and visit residents each work day.
• Work under the direction of the director of nursing to assure that all nursing personnel is complete.
• Responsible for nursing assessment as needed, documenting assessments, notifying physicians and families of change in condition, obtaining appropriate orders and implementing orders.
• Communicate with facility administrator and director of nursing.
• Available for call when needed by members on duty in matters pertaining to nursing care.

• Graduate of an approved school of nursing.
• Current LPN license.
• Maintain current state registration and ongoing education.
• Basic knowledge over Home Health.
• Detail oriented and able to maintain accuracy.
• Ability to maintain confidentiality related to facility and residents information.
