• Evaluate patients in their homes and develop individualized care plans
• Provide skilled occupational therapy interventions focused on ADLs, mobility, safety, and independence
• Educate patients and caregivers on adaptive strategies and equipment
• Collaborate with an interdisciplinary care team
• Complete accurate and timely documentation in compliance with regulations

• Active Occupational Therapist license in DC, VA, or MD
• Minimum one year of occupational therapy experience with home health preferred
• Strong clinical judgment and communication skills
• Reliable transportation and willingness to travel within assigned areas
• Compassion, professionalism, and a patient centered mindset
