Nurse Practitioner - Arlington

We are seeking a Part time Advanced Practice Nurse Practitioner to join our growing House Calls PCP practice to see patients in Arlington, Grand Prairie, Mansfield territory. Applicants must live near the locations posted and able to commute to the patients homes. 1 year experience as a NP caring for Geriatric patients, preferably in the home environment is required. Only qualified candidates will be considered.

• Performing physical exams and patient observations for Geriatric patients in their home, ILF, ALF, Memory Care, Group Homes and Senior Communities.
• Recording patients' medical histories and symptoms in EMR system.
• Creating patient care plans and contributing to existing ones.
• Ordering, administering, and analyzing diagnostic tests.
• Identifying the need for and ordering medical equipment.
• Diagnosing and treating complex health issues.
• Managing, ordering, assessing effectiveness of medications and other treatments.
• Detecting changes in patients' health and modifying treatment plans, as needed.
• Consulting with healthcare professionals and families.
• Training patients and their families to manage and prevent illness and injury.
• MIPS / Quality Measures.
• NO ON CALL.
• NO WEEKENDS.
• NO MAJOR HOLIDAYS.
