ResponsibilitiesStrategy / Operations – Responsible for the effective daily operation of the care transition function. Responsible for providing and/or implementing strategies to improve operations and outcomes. Determines proactive strategies and methods necessary for a high-performance culture related to care transition management. Utilizes change management techniques to facilitate leader and staff adoption of workplace changes. Directs the activities of the Care Transition Managers (RN & SW).
Serves as a content expert on compliance, mitigation activities, documentation, clinical and transition documentation, psychosocial issues, child and adult protective care services, adoptions, complex behavioral health cases, community resource needs, guardianship processes, mental health treatment declaration, indigent programs, and more.
Develops and oversees the implementation of policies, procedures, and standards that provide structure for care transition management. Participates in the development of new staffing and care coordination designs that support system goals. Supports standards that are consistent with hospital, community and nationally recognized evidence-based practice. Participates in collection and analysis of clinical and financial data and makes recommendations to improve performance.
Achieves and maintains high levels of internal and external customer satisfaction. Develops and manages the operating budget as required by THR.
Leadership – Responsible for providing overall direction and oversight to the care management function to obtain expected outcomes. Directs the activities of the organization related to Care Transition Management. Provides input into the strategic planning processes for THR and medical staff. Participates in appropriate THR and community forums. Achieves positive results for the organization related to investigations of complaints requiring the Director’s direct involvement.
Facilitates as lead staff at various organizational level meetings. Provides educational offerings on new tools, techniques and approaches to a wide variety of audiences. Supports and promotes community health activities among staff and colleagues. Maintains excellent professional relationships with system and entity leadership teams. Communicates and collaborates with THR in the areas of data and performance improvement, care coordination, medication management, and social services.
Human Resources – Develops and implements strategy to create a positive employee relations environment. Supports recruitment, orientation and retention of competent staff. Mentors growth of managers & staff. Oversees recruitment and retention program. Promotes professional growth and development. Coaches and mentors staff to improve performance. Identifies leadership talent and develops leadership skills. Achieves and maintains high levels of employee satisfaction. Establishes and maintains relationships with medical staff.
Fiscal Responsibility – Develops annual capital and operational budget and monitors compliance. Considers clinical and operational outcomes and financial implications. Evaluates new technology or products for cost effectiveness. Reviews budgets and justifications. Develops action plans to address variances. Utilizes management reports to evaluate budget and staffing variations. Resolves daily staffing issues.
Customer Satisfaction – Incorporates customer needs and expectations into daily operations. Promotes team building strategies. Reviews customer satisfaction evaluations to determine gaps. Takes appropriate action to remedy gaps. Reviews and discusses patient, staff, physician, and department complaints.
Qualifications- Education – Bachelor’s Degree in Nursing (Required) or Master’s Degree in Social Work (Required)
