We are seeking full‑time Social Workers to join our dedicated team of 2, supporting patients and families with compassionate, high‑quality care as we expand across Texas.

As a full‑time Social Worker, you will provide coordinated, in‑home care for patients of all ages, conducting psychosocial assessments, counseling, and assisting with funeral arrangements, death benefits, support groups, community resources, and referrals to address the needs of patients and their families. You’ll play a key role in the coordination of care and support.

• Education:
  
  Master’s or Doctoral degree from an accredited School of Social Work
• Licensure:
  Social Work certification and a current driver’s license
• Experience:
  
  Minimum of 1 year of experience as an MSW in a hospice setting
• Skills:
  
  Strong knowledge of community resources, excellent interpersonal skills, and current CPR certification
• Transportation:
  Reliable transportation and valid auto liability insurance
