The Post Transition Clinical Care Manager provides direct-service and clinical oversight to Colbert and Williams class members that have transitioned out of nursing facilities. Provide care coordination and clinical community-based interventions for mental health and/or physical health needs. Create comprehensive wraparound service plans and coordinate and monitor services. The Post Transition CCM will focus on one of two areas: KYC Team Services, or Collar Counties.
The CCM-Post Transition for Collar Counties will carry a caseload of Colbert and Williams Class Members who primarily do not have primary mental health services with KYC, and reside in the Collar Counties (Lake, McHenry, DuPage, Kane) and may include Northern Cook County based on caseload needs. The CCM will work collaboratively to coordinate services with providers serving assigned class members.
Treatment services are delivered at flexible times that meet clients’ needs and in settings that offer the best potential for positive outcomes.
- Meet regularly with Class Members in the community in the KYC service area around the Northwest Suburbs of Chicago, primarily focusing on Collar Counties of Lake, McHenry, DuPage, and Kane.
- Oversee and coordinate to ensure quality and access to all recommended services for Colbert and Williams Class Members that have transitioned out of nursing facilities and are assigned to non-KYC service providers.
- Coordinate with the Clinical Care Managers throughout the discharge planning process for Class Members in the pre-transition stage to ensure continuity of care after nursing home discharge.
- Take initiative and be accountable for and manage all aspects of post-transition, including being able to navigate changes at the system or individual level and advocate for Class Members and resolve conflicts.
- Review transition plan to ensure all aspects are being implemented.
- Provide comprehensive, integrated service plans and connect to services to help clients improve, regain, and develop skills needed for daily life focused on community mobility and integration, cognitive processes, support with engagement in IADLs/ADLs, hygiene and self-care, sensory processing, safety with transfers, fall prevention, and adaptive equipment to increase safety and independence.
