: $19.89 - $28.84. The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

    Monday – Friday (8:30a – 5p), Hybrid position.
What You Will Do

The Transitions of Care Navigation Program follows high-risk patients discharged from Inpatient, Observation, or Emergency Department. Patients are contacted telephonically up to thirty (30) days post-discharge. The program objectives are to decrease all-cause readmissions within thirty (30) days after discharge and to improve timely follow-up after an acute exacerbation of a chronic condition by engaging patients in their care.

Transitions Of Care Clinical Responsibilities
  • Support lower risk patient population and as needed support the MSW or RN with the higher risk patients
  • Telephonic outreach to patients to complete post discharge initial assessment
  • Support with connecting patients to resources
  • Support patients with appt scheduling and coordination of care post hospitalization or ED discharge
  • Monitor tasks in portal for patients that require outreach and support with coordination of care
What You Will Need
  • Education:

    Preferred current certification from a national Medical Assistant certification body or obtain within 3 months of hire or transfer.
  • Certification: BLS/CPR Certification for the Healthcare Provider required
  • Experience:

    Working knowledge of and experience with the use clinical equipment and materials.
