Position: Assessment Specialist (Transition)

Assessment Specialist (Transition) – Join the Kenneth Young Center team to help adults transition from nursing facilities to community living.

Kenneth Young Center is a 501(c)(3) nonprofit dedicated to comprehensive outpatient behavioral health services in the Northwest suburbs of Chicago. We offer outpatient therapeutic care, recovery‑oriented support, community prevention, LGBTQ+ outreach, older adult services, and crisis intervention.

Compensation & Benefits

Responsibilities
  • Complete comprehensive strength‑based, client‑centered, trauma‑informed clinical mental health assessments (IM‑CANS) for adults in skilled nursing facilities and specialized mental health nursing facilities.
  • Conduct client interviews, conduct family consultation, and review records to develop Transition Service Plans (CSPs).
  • Act as LPHA for staff in the Behavioral Health division by reviewing IM‑CANS/IATP and providing feedback.
  • Collaborate with KYC team members, nursing facility staff, and local community mental health services to coordinate care.
  • Monitor referrals and complete assessments within 24 hours to meet grant requirements.
  • Maintain high‑quality documentation and compliance, ensuring all notes and assessment plans meet organizational standards.
  • Ensure follow‑up of assessments and that IM‑CANS are signed within 14 days.
  • Meet or exceed productivity targets estimated at 50‑60 %.
  • Participate in clinical and administrative supervision, training, and performance evaluations.
  • Submit schedules, timesheets, mileage, and other required reports accurately and on time.
Qualifications
  • Preferred Master’s Degree in Social Work, Psychology, Counseling, or closely related discipline.
  • Strongly preferred LPHA in Illinois (LCPC/LCSW), licensed to practice independently.
  • At least 1 year of experience completing comprehensive mental health assessments for adults with serious mental illness.
  • Demonstrated experience working with individuals transitioning out of facilities, chronic homelessness, complex medical issues, and/or substance use difficulties.
