What to Expect (Job Responsibilities):
  • Serve as the primary point of contact for members, coordinating assessments and service plans
  • Facilitate the person-centered planning process and ensure appropriate service delivery
  • Consult with members and families regarding health care needs and benefits
  • Complete health care questionnaires and behavioral assessments to inform care plans

    Document care coordination activities and maintain accurate records in compliance with guidelines
What is Required (Qualifications):
  • High School Diploma or GED equivalent required
  • Minimum of one (1) year of experience working with developmentally or intellectually disabled or behavioral health clients
  • Must have a valid driver’s license, vehicle, and verifiable insurance
  • Employment conditional upon successful clearance of a criminal background check and pre-employment drug screen
  • Intermediate proficiency level with Microsoft Office, including Outlook, Word, and Excel
How to Stand Out (Preferred Qualifications):
  • Experience in case management, care coordination, or home/community health management
  • Strong understanding of local, state, and federal healthcare laws and regulations
  • Ability to communicate effectively with diverse individuals and communities
  • Familiarity with community and state support resources
  • Proven organizational and time management skills
