Position: Community Based Care Coordinator I Must Reside in Green County

Responsibilities
  • Serve as the single point of contact for members, facilitating assessments and service coordination
  • Develop and monitor Person-Centered Service Plans in collaboration with members and stakeholders
  • Conduct ongoing communication with the internal clinical team and community members
  • Document care coordination activities and ensure compliance with accreditation guidelines
  • Perform regular member visits and provide health education and wellness materials
Qualifications
  • High School Diploma or GED equivalent required
  • Minimum of one (1) year of experience working with developmentally or intellectually disabled or behavioral health clients
  • Intermediate proficiency in Microsoft Office, including Outlook, Word, and Excel
  • Valid driver’s license, vehicle, and verifiable insurance
  • Successful clearance of background checks and pre-employment drug screen
Preferred Qualifications
  • Experience in case management, care coordination, or home/community health management
  • Strong understanding of local, state, and federal healthcare laws and regulations
  • Knowledge of community and state support resources
  • Excellent organizational, decision-making, and problem-solving skills
  • Ability to communicate effectively with diverse populations
