A community health worker is a front‑line public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.

- Education: High School Diploma or equivalent
- Experience: 1-3 years CHW experience
- Certification/Registration: Basic Life Support
- Other Skills and Knowledge: Strong relationship and collaboration skills, Excellent communication skills, Ability to work independently, Compassion for poor and underserved members of the community, Epic electronic medical record
- Certification/Registration: Community Health Worker Certification if available
- Other Skills and Knowledge: Additional language skills as applicable to community
