Position: Community Based Care Coordinator I - Must reside in Washington County
Location: Helena-West Helena

Overview

What to Expect (Job Responsibilities)
  • Serve as the single point of contact for members, coordinating assessments and service planning
  • Facilitate person-centered planning and assist with care coordination activities
  • Monitor service delivery and ensure compliance with care standards
  • Conduct member visits and community outreach to educate and support members
  • Maintain accurate documentation of care coordination activities and member interactions
What is Required (Qualifications)
  • High School Diploma or GED equivalent required
  • Minimum of one (1) year of experience working with developmentally or intellectually disabled or behavioral health clients
  • Intermediate proficiency with Microsoft Office, including Outlook, Word, and Excel
  • Valid driver’s license, vehicle, and verifiable insurance
  • Successful clearance of a criminal background check and pre-employment drug screen
How to Stand Out (Preferred Qualifications)
  • Experience in discharge planning, case management, or home/community health management
  • Strong understanding of local, state, and federal healthcare laws and regulations
  • Ability to communicate effectively with diverse populations
  • Knowledge of community and state support resources
  • Strong organizational and time management skills
