Summary- Position type:
Non‑exempt, full‑time or part‑time, hourly role. - Scope of role:
Provide direct support within a single program or unit, typically supporting 4–6 individuals in residential settings or up to 10 individuals in periodic programs. - Key responsibilities:
Deliver support per individual service/program plans; assist with daily activities (personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living).
- Direct support:
Assist with socialization, behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living. - Records:
Accurately document progress and activity; review records and logs; maintain confidentiality. - Management of individuals’ assets:
Make cost‑effective purchases within budget and according to individual preferences. - Relationships:
Maintain healthy professional relationships with individuals, families, guardians, and case managers; greet family members and visitors; implement Sevita customer service standards. - Reporting:
Report any allegations of abuse or neglect and any incidents in accordance with Sevita policy and external requirements. - Rights:
Maintain confidentiality, respect the rights of persons served, practice universal precautions, and assist individuals in exercising their rights. - Health care:
Appointments, medication administration, general health care, medical supplies and equipment, meal preparation – all to be performed accurately and with safety in mind. - Employment responsibilities:
Attend orientation and ongoing training, volunteer for staff meetings, adhere to scheduling, maintain teamwork, and comply with workplace safety policies. - Maintenance:
Transport individuals, perform housekeeping, make minor repairs, maintain safety, and report any hazards. - Other:
Perform other duties as required and use Mobile Punch Authorized for timekeeping.

Education and Experience
- High school diploma or equivalent preferred.
- Six months of experience in human services preferred.
- Must be 18 years of age.
- Working knowledge of computers.
- Valid driver’s license in good standing.
- Car registration and vehicle insurance if providing transportation for individuals receiving services.
- All state‑required training and certification completed within mandated time frames.
- Maintain valid driver’s license, meet all insurance requirements, and be approved by management to be a driver for the company.
- Works at the program location and may accompany individuals into the community.
- Heavy work:
Exert up to 100 pounds of force occasionally, 50 pounds of force frequently, and 20 pounds of force constantly to move objects.
- Additional state‑specific requirements and policies are referenced in the CS New Jersey addendum.
- Employees must cooperate with licensee and Department of Human Services staff during inspections, inquiries, or investigations.
- All employees must obtain drug testing, criminal background checks, and other required screenings before and during employment.
- Employees at least 18 years of age are required to comply with all training and proficiency demonstrations.
