Role Overview :
Psychiatric‑Mental Health Nurse Practitioner — Part Time, Richmond, VA

Legacy Care is seeking a dedicated and experienced Psychiatric‑Mental Health Nurse Practitioner (PMHNP) to join our team on a part‑time basis in Richmond, VA. The successful candidate will provide comprehensive psychiatric assessment, diagnosis and management for residents in post‑acute and long‑term care facilities. This position includes medication management, brief therapeutic interventions, collaboration with interdisciplinary teams and participation in quality improvement activities to support residents’ mental health and overall wellbeing.

Key Responsibilities — PMHNP, Post‑Acute Care
  • Conduct psychiatric evaluations and mental health assessments for residents in post‑acute and long‑term care settings
  • Diagnose psychiatric disorders and develop individualized treatment plans incorporating pharmacotherapy and non‑pharmacologic strategies
  • Manage psychiatric medications, monitor for efficacy and side effects, and coordinate medication adjustments with facility staff and primary care clinicians
  • Provide brief therapeutic interventions and crisis management as appropriate within the scope of practice
  • Collaborate with nursing staff, physicians, therapists, social workers and facility leadership to integrate mental health care into the resident’s overall care plan
  • Participate in facility rounds, care‑planning meetings and interdisciplinary case conferences
  • Document psychiatric encounters accurately and promptly in the Electronic Medical Record (EMR)
  • Support medication safety, psychotropic stewardship and deprescribing initiatives when appropriate
  • Provide clinical consultation and education to facility staff on behavioral health management, de‑escalation techniques and trauma‑informed care
  • Maintain professional standards of practice, including infection control, documentation and regulatory compliance
Essential Qualifications And Skills — PMHNP, Richmond VA
  • Master’s degree in nursing (MSN) or Doctor of Nursing Practice (DNP) with Psychiatric‑Mental Health Nurse Practitioner specialty from an accredited institution
  • Active and unrestricted Virginia APRN and nurse practitioner licensure (or eligibility for licensure)
  • Prescriptive authority as required by Virginia state regulations
  • PMHNP certification from a recognized certifying body preferred
  • Clinical experience in behavioral health, geriatrics, post‑acute care or long‑term care preferred
  • Proficiency with Electronic Medical Record systems and clinical documentation
  • Strong clinical judgement, interpersonal and communication skills; ability to work effectively within multidisciplinary teams
  • Commitment to person‑centered care, cultural sensitivity and ethical practice
