Position: PHYSICIAN-FAMILY PRACTICE
Location: San Vicente
  • Develops and implements a plan of care for patient with focus on prevention and early detection in accordance with accepted healthcare guidelines.
  • Provides healthcare for patient within the framework of established guidelines and protocols. This includes developing patient care plans as necessary and performing the necessary follow up.
  • Provides information and feedback to patients on a timely basis, enabling them to assume responsibility for their own health.
  • Maintains complete, accurate and up-to-date medical records, documenting all aspects of care given.
  • Makes referrals as necessary and works with Centro San Vicente staff and other healthcare providers to assure continuity of care and multidiscipline healthcare.
  • Functions as a preceptor for medical students and assists with the health education programs for Centro San Vicente staff (only MD and DO). Other health professionals
    - Midwives, PA, and FNP.
  • Collaborates with other physicians in the area.
  • Represents the mission of Centro San Vicente and the Daughters of Charity to patients and visitors.
  • Participates in the process for finalizing the Health Care Plan in the determination of strategies, measures, and outcomes that are desired.
  • Provides on call coverage on a rotational basis.
  • Works to maintain a high level of patient satisfaction to reflect on the patients’ satisfaction staff.
  • Maintains at least 80% attendance at provider meetings.
  • Promotes teamwork, creating a good work environment.
  • Maintains proficiency in coding patient visits and in documenting Medical Necessity.
  • Maintains regulatory requirements, including all state, federal and JCAHO regulations.
  • Attends annual review and performs departmental in-services.
  • Attends committee, CQI and management meetings, as appropriate.
  • Resolves personnel concerns at the department level, utilizing the grievance process as required.
  • Ensures compliance with policies and procedures regarding department operation, fire, safety, and infection control.
  • Effectively and consistently communicates administrative directive to personnel and encourages interactive department al meetings and discussions.
  • Meets volume and/or RVU requirements as per national FQHC standards.
  • Maintains chart documentation as per PI Quarterly evaluations.
  • Maintains and updates medication and problem list as necessary.
  • Evaluates and documents pain when appropriate.
  • Evaluates and addresses barriers to care.
  • Provides assistance with psychosocial care/social work.
  • Communicates and demonstrates the mission, ethics and goals of the facility.
  • Performs other related duties as assigned.
Knowledge, Skills and Abilities:
  • Basic computer knowledge.
  • Well-developed verbal and written communication skills.
  • Ability to make independent decisions without on-site supervisor.
  • Culturally sensitive to the population served by a Community Primary Health Care Center.
  • A holistic philosophy of health care that integrates the biological, emotional, spiritual and social aspects.
  • Ability to accept and incorporate faith based approaches into the medical practice philosophy.
  • Ability to work a flexible, extended, work schedule including attendance at meetings and workshops.
  • A sense of compassion for the indigent.
  • Ability to work at the time and location assigned.
Minimum Qualifications:
  • Graduate from an accredited medical school, postgraduate internship, and residency training.
  • Board eligible or Board Certified in Primary Care, (i.e. Family Practice, Pediatrics, and Internal Medicine).
  • Current Texas driver’s license and ability to meet all qualifications to be covered in Texas for Professional Liability Insurance.
  • Experienced in community based primary care preferred.
  • Experience in the clinically-related management aspects of primary care administration including cost efficient clinical management, medical policy development and clinical outcome measures.
  • Experience in community-oriented or population-based medicine and requirements for Joint Commission for the Accreditation of Health Care Organizations.
