** Job Summary
** The
** Licensed Vocational Nurse (LVN)
** provides high-quality patient care in the home setting by following the established
** plan of care
** under the supervision of the Registered Nurse. This role is ideal for LVNs who enjoy one-on-one care, patient education, and helping individuals recover after orthopedic and surgical procedures.
** Key Responsibilities
*** Deliver nursing care as directed by the RN and patient plan of care
* Provide accurate, timely, and compliant documentation
* Collect assessment data and communicate findings to RN and physicians
* Collaborate with the interdisciplinary care team
* Use clinical equipment safely and efficiently
* Engage in ongoing professional development
** Requirements
* ** Active
** California LVN License
*** Current
** CPR Certification**
* ** 2+ years of direct patient care experience** (home health experience required)
* Valid driver’s license, reliable insured vehicle, and good driving record
