Our Certified Community Behavioral Health Clinic (CCBHC) program is a specially-designated program to ensure access to coordinated, comprehensive behavioral health care. It serves anyone who requests care for mental health or substance use, regardless of their ability to pay, diagnosis, or age. The clinic provides integrated behavioral and physical health services so people of all ages can receive comprehensive care without needing to piece together support across multiple providers.

Care coordination helps patients navigate systems to address social drivers of health, and we provide access to crisis services 24/7.

Program Description

Pacific Clinics' CCBHC program operates in the Los Angeles vicinity, with virtual work opportunities. Our model of care includes comprehensive behavioral and physical health services, care coordination, and crisis services available 24 hours a day, 7 days a week.

Compensation We Offer
  • The initial compensation for this position ranges from $69,971.00 – $86,055.00 annually.
  • Salary depends on experience above the minimum qualifications and internal equity considerations.
  • Salary may vary if you reside in a different location than the location posted.
  • 8% License Differential for LMFT, LCSW, LPCC, PsyD, and select Peer Certifications.
  • 7.5% Bilingual Differential for qualified positions (Spanish approved).

Job Overview

Works within the values and clinical philosophy of the agency and within the Certified Community Behavioral Health Clinic framework and Collaborative Care Model. The Behavioral Care Manager (BCM) is an experienced clinician who manages coordination with the care team which includes the patient, primary care providers, psychiatric consultant, and system navigator. The BCM is responsible for assessment, diagnosis, treatment planning, medication monitoring and support, and psychotherapy as needed, using evidence‑based, brief solution‑focused interventions for patients with mild to moderate behavioral health needs.

Responsibilities
  • Support the mental and physical health care of patients on an assigned patient caseload. Closely coordinate care with the patient’s medical provider and other mental health providers and system navigators when appropriate.
  • Screen and assess patients for common mental health and substance abuse disorders. Facilitate patient engagement and follow‑up care.
  • Provide patient education about common disorders and available treatment options.
  • Systematically track treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications, via in‑person or telehealth.
  • Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence, side effects, and effectiveness.
  • Provide brief behavioral interventions using evidence‑based techniques such as behavioral activation, problem‑solving treatment, motivational interviewing, or other appropriate methods.
  • Provide or facilitate referrals to evidence‑based psychosocial treatments (e.g., problem‑solving treatment or behavioral activation) as clinically indicated.
  • Participate in regularly scheduled consultations (daily or weekly) with the psychiatric consultant, communicate recommendations to the patient’s medical provider, and focus on patients who are not improving as expected.
  • Track patients’ follow‑up and clinical outcomes using a registry, document encounters, and re‑engage patients as needed.
  • Document patient progress and treatment recommendations in EHR and other required systems to share with providers.
  • Facilitate treatment plan changes for patients not improving as expected, in collaboration with the medical provider and psychiatric consultant.
