Position: Case Manager/Social Worker Family Find (part-time)

Overview

Outagamie County is seeking a Part-time Case Manager/Social Worker to provide professional social work services to individuals and families, helping them overcome personal, family, health, and economic problems. The position is in the Initial Assessment Unit with primary responsibilities for family find, with the potential to assist in other areas.

Hours

Part-time, 24 hours per week, 3 days per week (generally 8:00 am – 4:30 pm; two days include Wednesday and Friday). Work hours are flexible to meet client needs; evenings may be required. On-call coverage is required for this position. Interview discussions will address specific hours.

Key Responsibilities

The following duties are the most critical for this position. They are not exclusive or all-inclusive. Other duties may be required and assigned.

  • Assess reports to determine validity of the report and legal basis for involvement, observing and interviewing family members and collateral contacts.
  • Determine, through interviews, assessments, home visits, investigations, etc., the range of services needed by individuals and families; explain services and discuss client rights and responsibilities.
  • Recommend services from other resources; arrange for appointments and establish connections with providers to meet client needs.
  • Maintain case histories and complete appropriate paperwork and data tracking according to department policy; maintain client confidentiality by following HIPAA and Agency Confidentiality policies.
  • Provide court-related services including preparing and writing court reports, court conditions, testimony, and other related paperwork.
  • Assist clients in developing inner strengths and maximizing informal supports and resources.
  • Foster client growth by interpreting attitudes and patterns of behavior; explain and identify new options and be strength-based in exchanges.
  • Monitor case plans and provide feedback to clients and other interested parties on follow-up actions required.
  • Attend necessary trainings related to this position.
  • Provide on-call services as assigned.
Education/Certifications/Experience
  • Must possess and maintain a valid driver’s license.
  • Documentation of car insurance.
  • Case Manager:
    Bachelor’s Degree in Social Work or a human services related field.
  • Social Worker:
    Bachelor’s Degree or Master’s Degree in Social Work or a human services related field and an active Wisconsin social work certification.
Required or Preferred Skills
  • Knowledge of and ability to apply social work principles and methods, including human development, developmental psychopathology, treatment planning, AODA concerns and treatment, and working with vulnerable populations.
  • Maintain objectivity and a positive outlook while dealing with negative situations, using tact and diplomacy while maintaining professionalism.
  • Maintain accurate records and comply with grant reporting and agency documentation policies; maintain confidentiality.
  • Work independently, make timely decisions, and know when supervision is necessary.
  • Manage time effectively; stay organized and prioritize tasks to meet paperwork needs and agency hours; be punctual, reliable, and dependable.
  • Manage conflict through motivational interviewing, conflict resolution, and open, transparent communication.
  • Know how to use computer software and equipment.
  • Comply with safety regulations including installing car seats.
  • Communicate in English; follow written and oral instructions; excellent writing skills.
  • Physical ability to lift, carry, push, pull, or move objects up to 20 pounds with proper body mechanics.
  • May be required to work extra hours and perform other duties as assigned by supervisor/manager.
  • Respond to Agencies/Divisions Emergency/Disaster Plan.
