Registered Nurse Care Manager Float (RNCM Float) – Appleton, WI

The Registered Nurse Care Manager Float (RNCM Float) acts as a temporary member of the interdisciplinary team (IDT) serving frail elders, adults with physical disabilities, and adults with intellectual/developmental disabilities who are members of Lakeland Care (LCI). The team’s goal is to support LCI members in navigating health systems and utilizing resources to promote optimal health and wellness by providing high‑quality, person‑centered, outcome‑based care.

Position Summary & Performance Outcomes

Under the general direction of the Care Management Staffing Supervisor, the RNCM Float provides nursing care management in partnership with a Care Manager (CM). The RNCM Float coordinates and designs provision of services and supports based on a comprehensive assessment of the member’s identified outcomes and needs. The RNCM Float monitors and evaluates the effectiveness of health‑related services based on the member‑centered plan per LCI policy and procedures and Department of Health Services (DHS) contract requirements.

The IDT provides member education to minimize risk, maximize independence, and improve health, safety, and quality of life.

The RNCM Float position covers RNCMs during periods of extended leave, interim periods while a new employee is hired or trained, or other reasons as determined by the CM Supervisor/Program Manager. The RNCM Float coordinates shadowing opportunities and warm hand‑offs as caseload transitions occur.

Responsibilities & Competencies
  • Conduct a comprehensive assessment of the member’s outcomes, needs and risks; reassess as the member’s condition changes.
  • Coordinate acute and primary care services, care transitions and related follow‑up care.
  • Develop, coordinate, monitor, and evaluate members’ outcome‑based member‑centered plans, considering cost and effectiveness in authorizing services and choosing providers.
  • Promote prevention, wellness, and chronic condition management by incorporating preventative, supportive, restorative, and rehabilitative care into the care planning process.
  • Manage workload and caseload logistics using strong time‑management skills.
  • Communicate effectively with members and all involved parties to ensure a comprehensive understanding of roles in the care planning and decision‑making process, promoting maximum independence.
  • Establish relationships with LCI teams, the assigned CM Supervisor, and support divisions during coverage.
  • Be flexible with travel time and able to cover multiple service regions as required.
Requirements
  • Current license to practice as a Registered Nurse in the State of Wisconsin.
  • Bachelor’s degree in nursing strongly preferred.
  • Two (2) or more years of skilled nursing experience preferred, ideally in gerontology, disability, or home care.
  • Demonstrated proficiency in applying the Family Care principles and the Resource Allocation Decision process to practice.
  • Adaptable, able to prioritize and work in a fast‑paced environment.
  • Working knowledge of computers, computer programs, typing, and data entry.
  • Ability to access members’ homes; compliance with ADA regulations is required.
  • Ability to lift up to 25 lbs.
  • Current driver’s license, acceptable driving record and proof of adequate insurance.
