Job Description
The Case Coordinator reports to the program supervisor and is responsible for providing support to participants, customers, and clients in alignment with the division's objectives and goals.
Key Responsibilities- Gathering information from clients, family members, and other agencies to assess their needs.
- Providing individuals and families with relevant information to meet their goals and preferences in a person-centered and culturally sensitive manner.
- Building and maintaining relationships with other human services, medical, and social services agencies.
- Identifying gaps in service delivery and participating in program development initiatives.
- Attending meetings as required by the unit/division.
- Maintaining accurate records and preparing reports for case coordination.
- Screening referrals for children with disabilities.
- Providing ongoing case management and service coordination to families and children.
- Assessing needs and offering information and services using a person-centered approach.
- Providing support, education, and technical assistance to families, individuals, and agencies.
- Maintaining documentation for court proceedings and completing timely documentation within three business days.
- Maintaining regular attendance and working overtime as needed.
- Performing other duties as assigned.
- High school diploma or equivalent required;
Associate's or Bachelor's degree in a related field (e.g., social work, psychology, human services) preferred. - Certification as a Social Worker is desirable.
- At least two years of experience in the health or human services field is preferred.
- Must have a valid driver's license.
- Knowledge of and adherence to goals, policies, and procedures.
- Proficiency in operating office equipment.
- Ability to travel efficiently.
- Commitment to professional ethics and standards.
- Strong organizational skills and ability to manage paperwork effectively.
- Ability to perform basic mathematical calculations and use descriptive statistics.
- Proficiency in preparing various documents.
- Effective communication skills.
- Ability to work well in a team environment.
