JOB REQUIREMENTS:
A wonderful family in the Appleton, WI area is eagerly seeking a part-time, in-home caregiver for their granddaughter with disabilities. This position is vital in supporting this young girl and her family to continue to thrive in their home and community. This is a pet-friendly house; no care for the animals. The ideal candidate is comfortable providing hands-on personal care assistance and working with an individual who is non-verbal.
Schedule:
the family is hoping to find someone who can work 10 hours a week. The hours are very flexible, and the family is open to discussing availability. Bonus points if you have flexibility in your schedule for last-minute needs. Pay: $17.97/hour.
- Assisting with personal care tasks such as incontinence care, teaching age-appropriate independence, and providing safety and supervision.

Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children's Long-Term Support (CLTS) program. Self-direction allows participants to make their own choices, selecting who provides their care and how those services are delivered. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork. Our mission is to help individuals of all ages navigate self-directed care with connection, coordination, and compassion.
High schoolers are welcome to apply. Someone who has ABA experience is a plus. Able to understand that the child does communicate, but with short language. Friendly and kind. Has a valid driver's license and a reliable vehicle. Unwavering patience. Experience with autism is preferred but not required. Experience working with children, including work with siblings. Ensuring safety and engagement. Able to pass a Wisconsin background check.
