Case Management Nurse:
Coordinate long‑term care for patients, treat them at optimal times to keep them healthy, and reduce hospital utilization. Manage specific patient groups, research treatments, and work with insurance companies to secure the best cost‑effective care.

• Coordinate long‑term care plans for patients.
• Specialize in treating diseases such as HIV/AIDS or cancer, or in specific age groups (geriatrics, pediatrics).
• Research the latest treatments and procedures.
• Collaborate with insurance companies to facilitate optimal, cost‑effective care for patients.

• Minimum of 1 year of current work experience in Case Management.
• California State Healthcare Provider license or willingness to obtain one.

• BLS (Basic Life Support)
