Exciting Case Management Opportunity for Registered Nurses!Job Description:
- Start Date:
ASAP
- Shift: Day 5x8-Hour (08:00 - 16:30) - Patient Ratios:
Days 1:25, Weekends 1:45
Key Responsibilities:
- Engage in acute care case management - Collaborate on discharge planning and care coordination - Conduct assessments of patients to develop tailored treatment plans - Organize and participate in family meetings - Manage needs assessment, prior authorizations, and benefits eligibility - Coordinate DME, SNF, and ambulance transport arrangements independently
Required Experience:
- 2 years in Acute Care Case Management/Discharge Planning - Strong understanding of CMS, regulatory requirements, and DRG processes
- Experience with HMOs, IPAs, and crisis management - Comfortable working with diverse patient age groups:
Neonates, Adolescents, Adults, Geriatrics Essential
Skills:
- Proficient in computerized documentation (Epic) - Knowledge of various treatment options, including drug/alcohol abuse and mental health crisis intervention - Ability to work collaboratively with interdisciplinary support teams, including Physical Therapy and Social Services Scheduling Details:
- Weekend Rotation:
Every other weekend required - Maximum Pre-Approved Time Off: 1 Request - Orientation
