Lone Tree Post Acute — Antioch, CA (99-bed skilled nursing facility)
Position Type- Employment Type: Full-Time, Part-Time, or PRN

- Starting Rate: $45-$50 per hour (dependent on experience and position)

The Staff Occupational Therapist Assistant assists the Staff Occupational Therapist with patient-related activities and direct patient care.
Essential Duties- Treat patients as directed by the Occupational Therapist.
- Record daily treatment notes and weekly progress notes per OT Board requirements.
- Implement the physician's treatment plan for patients.
- Assist the nursing department with training Restorative Aides.
- Communicate with supervisors and other healthcare team members regarding patient progress, problems, and plans.
- Participate in in-service training programs for other staff in the facility.
- Maintain accurate documentation, including evaluations, daily treatment notes, weekly progress notes, and recertification documentation as per Occupational Therapy Board regulations and other payers' requirements.
- Record and communicate treatment changes in accordance with policy and procedures.
- Educate patients' families or nursing staff on maintenance programs and provide caregiver training in preparation for discharge from therapy services.
- Participate in discharge planning.
- Assist in cleaning and maintaining the treatment area.
- Report any issues with department equipment to ensure it remains in good working order.
- Build positive relationships with patients/residents, families, staff, and professional colleagues to facilitate effective communication and care delivery.
