Full Time Physical Therapist Case Manager

Anticipated Territory: Antioch Area

In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their home environment.

Your goals as a Physical Therapist are to restore function to prevent or minimize disability following disease or injury and to help the client reach his/her maximum performance/function within the limits of his/her capabilities. You will evaluate and treat patients using medically prescribed physical therapy programs. You may supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards. Other responsibilities include:

• Perform an evaluation in the home and develop a care plan and work with PTAs to follow the care plan.
• Help patients with mobility, movement, and regain independence.
• Create detailed treatment and care plans for each patient.
• Communicate and coordinate with patients’ physicians to ensure a high level of care.
• Evaluate and report patients’ progress to the physician.

To be successful as a Physical Therapist:

• Current unrestricted CA Physical Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
• Understanding of development, behavior, and culture in health care
• Knowledge of state and federal homecare regulatory guidelines (preferred)
• Experience in home health care (highly preferred)
• Experience with high-acuity patients (a plus)
• Experience working with an interdisciplinary team (highly desired)
• Case Management fundamentals (highly desired)
• Self-directed, empathetic, critical thinker with outstanding interpersonal and leadership skills
• Organized

Wage range for Physical Therapist, Level 1: $120,000 – $135,000 plus up to $10,000 annual bonus, paid quarterly. Starting pay is commensurate with relevant experience. 21st Century offers generous bonus plans, comprehensive health benefits and 401k (up to 4% match).

As a practice, most PT Case Managers are hired as PTIs. Once eligible to supervise 2 PTAs, and take on a larger case load with greater autonomy, the individual will be promoted to PTII with a minimum salary of $135,000 plus a $20,000 quarterly bonus.
