Physical Therapist, PT (Healthcare)

Lone Tree Post Acute is a 99-bed skilled nursing facility located in Antioch, California. We are looking for a Physical Therapist to join our team.

Opportunities: Full Time, Part Time, and PRN

Job Responsibilities:
  • Develop effective treatment plans and obtain approval for services from referring physicians.
  • Treat patients according to the physician treatment plan.
  • Assist the nursing department with training of Restorative Aides.
  • Supervise Physical Therapy Assistants in patient care, following state practice act.
  • Communicate with supervisors and health team members regarding patient progress and plans.
  • Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
  • Conduct in-service training for staff.
  • Document evaluations, treatment notes, progress reports, and recertifications per PT Board State Practice Act and payer requirements.
  • Record treatment modifications as per policies.
  • Instruct families and nursing staff in maintenance and caregiver training for discharge planning.
  • Participate in discharge planning processes.
  • Recommend and facilitate ordering of durable medical equipment to promote patient independence.
  • Report equipment issues for maintenance.
  • Maintain positive and effective communication with patients, families, staff, and colleagues.
    • Minimum Requirements:
    • Valid Physical Therapist License
