Location Lone Tree Post Acute — Antioch, CA (99-bed skilled nursing facility)
Position Type

Compensation•
** Starting Rate:** $55-$65 per hour (DOE and position)

Key Responsibilities
* Evaluate patients, develop treatment plans, and obtain physician approval.
* Deliver physical therapy interventions consistent with the physician’s orders.
* Supervise Physical Therapy Assistants and train Restorative Aides.
* Document evaluations, daily notes, progress notes, and recertifications per state and payer requirements.
* Communicate patient progress and plans with the interdisciplinary team; attend care conferences and utilization reviews.
* Educate families and nursing staff on maintenance programs and caregiver techniques for safe discharge.
* Recommend and facilitate ordering of durable medical equipment (braces, walkers, etc.).
* Participate in in-service education for facility staff and report equipment issues promptly.
Required License / Certification
* Current, valid
** Physical Therapist (PT)
** license in California (or eligibility to obtain)
