Contra Costa County – Antioch Area.

In partnership with the Case Manager, provide skilled services to assist patients with breathing, speaking, and swallowing functions. Evaluate and treat speech, language, cognitive and fluency disorders that affect communication. Serve as an educator, patient advocate, and member of the interdisciplinary care team.

• Current unrestricted CA Speech Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of development, behavior, and the influence of culture on health care
• Knowledge of state and federal homecare regulatory guidelines (preferred)
• Experience in home health care (highly preferred)
• Experience with high acuity patients (a plus)
• Experience working with an interdisciplinary team (highly desired)
• Case management fundamentals (highly desired)
• Self-directed, empathetic, critical thinker, outstanding interpersonal skills
• Leadership, organizational abilities

Salary: $110,000 – $125,000 per year

Starting pay is commensurate with relevant experience. 21
