ResponsibilitiesThe Social Work Support Coordinator promotes a timely return to the community through the development of a comprehensive discharge and aftercare plan, incorporating utilization management principles, risk management principles, and continuity of care.
Responsibilities include:
- Assess patient needs, develop and coordinate care plans, and evaluate service effectiveness.
- Provide licensed social work case management coverage and assist with referrals for aftercare housing, psychiatric and other treatment services.
- Prepare discharge documentation, communicate with patients, and coordinate transportation and other case management activities as needed.
The employee works collaboratively with treatment team members and may provide other related case management activities as required.
Minimum Qualifications- Bachelor's degree in Social Work
- Licensure (LSW) in the State of Minnesota
- One (1) year of professional experience working with individuals with serious and persistent mental illness (SPMI)
- Knowledge of behavior modification principles sufficient to participate in planning and designing individual treatment programs for assigned clients
