Schedule: Full-time | 80 hours per pay period | Monday-Friday | 8 am – 4:30 pm

Whether creating personalized treatment plans, guiding clients through rehabilitation exercises, or collaborating with an interdisciplinary team, you’ll play a key role in helping clients achieve their highest level of independence and well‑being.

Responsibilities- Deliver Direct Client Care – Conduct comprehensive evaluations and develop personalized treatment plans with short‑term and long‑term goals.
- Maintain Accurate Client Records – Document services accurately and timely using Vivie’s EMR system, ensuring compliance with all payer sources and authorization requirements.
- Facilitate Follow‑Up Programs – Recommend and oversee continuation programs to support long‑term client success and optimize discharge outcomes.
- Collaborate and Communicate Effectively – Work closely with clients, families, physicians, and the interdisciplinary team to provide care.
- Ensure Regulatory Compliance – Complete and document supervisory visits with PTAs and RA/CNAs in accordance with state and regulatory guidelines.

- Current Physical Therapist license in the state of Minnesota.
- Strong collaboration and teamwork skills.
- Clear and compassionate communication abilities.
- Proficient in English for effective communication with staff, clients, and visitors.
- Successful completion of a state‑mandated background check.
- Able to physically perform the duties required by the position.
- Must obtain CPR certification within 6 months of hire.

No - This role does include supervisory visits with PTAs, but does not include formal supervisory responsibilities.
