Physical Therapist - Home Health - Anoka

Schedule: Full-time | 80 hours per pay period | Monday-Friday | 8 am – 4:30 pm

Whether creating personalized treatment plans, guiding clients through rehabilitation exercises, or collaborating with an interdisciplinary team, you’ll play a key role in helping clients achieve their highest level of independence and well‑being.

Responsibilities
  • Deliver Direct Client Care – Conduct comprehensive evaluations and develop personalized treatment plans with short‑term and long‑term goals.
  • Maintain Accurate Client Records – Document services accurately and timely using Vivie’s EMR system, ensuring compliance with all payer sources and authorization requirements.
  • Facilitate Follow‑Up Programs – Recommend and oversee continuation programs to support long‑term client success and optimize discharge outcomes.
  • Collaborate and Communicate Effectively – Work closely with clients, families, physicians, and the interdisciplinary team to provide care.
  • Ensure Regulatory Compliance – Complete and document supervisory visits with PTAs and RA/CNAs in accordance with state and regulatory guidelines.
  • Other duties as assigned – The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.
Qualifications
  • Current Physical Therapist license in the state of Minnesota.
  • Strong collaboration and teamwork skills.
  • Clear and compassionate communication abilities.
  • Proficient in English for effective communication with staff, clients, and visitors.
  • Successful completion of a state‑mandated background check.
  • Able to physically perform the duties required by the position.
  • Must obtain CPR certification within 6 months of hire.
