Job Overview
As a Registered Nurse in Home Health, you’ll provide compassionate and personalized skilled nursing care to clients in their own homes. You’ll deliver direct clinical care, educate clients and families, and ensure compliance with state and federal regulations, enhancing clients’ independence, safety, and well‑being.
Schedule:
PRN – minimum 3 shifts per month – Monday to Friday – 8 a.m. to 4:30 p.m., including on‑call/holiday rotation.
Pay: $40.00‑$49.00 /hr (hourly, non‑exempt) based on qualifications, experience and location.

Responsibilities- Provide direct client care: initiate preventive and rehabilitative nursing procedures, monitor and report changes in client conditions, educate and counsel clients and caregivers on care needs and support them in managing health challenges at home.
- Maintain accurate client records and billing: document all services using Vivie’s EMR system, demonstrate understanding of payer sources and authorization requirements, complete supervisory visits with LPNs and HHAs in compliance with regulatory guidelines.
- Ensure regulatory compliance and quality care: organize, develop, and direct skilled nursing services following nursing standards, state and federal regulations, and Vivie’s policies, maintaining quality, customer service, and fiscal responsibility.
- Communicate effectively and collaboratively: maintain clear and compassionate communication with clients, families, physicians, referral sources, and the interdisciplinary team, using exceptional verbal and written communication skills.
- Other duties as assigned: participate in team meetings, special projects, or assist in other areas to efficiently meet client needs.
- Current Registered Nurse (RN) license in the state of Minnesota.
- Cpr required within 6 months of hire.
- Valid driver’s license and access to an insured personal vehicle.
- Ability to pass a state‑mandated background check.
- Proficient in reading, writing, and speaking English.
- Physical capability to drive to client locations and provide hands‑on care.
