Salary: $60,000.00 – $73,000.00

• Represent the branch in external transitional care activities and maintain professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and other health groups.
• Develop patient‑centered transition plans by integrating clinical guidelines, protocols, and other metrics to promote quality and efficiency.
• Act as a public awareness representative and educate stakeholders about Enhabit services.

• Graduate of an approved nursing school.
• Licensed in the state of practice.
• At least one year of experience in home care.
• Demonstrated experience with federal, state, and local hospice regulations.
• Basic technology skills, including mobile device operation.

• Experience and knowledge in hospice care.

• Valid state driver’s license.
• Maintain automobile liability insurance as required by law.
• Responsible, dependable transportation in good working condition.
• Able to drive safely in all weather conditions.
