• Regularly re-evaluates patient nursing needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Uses health assessment data to determine nursing diagnosis.
• Provides health care instructions to the patient as appropriate per assessment and plan of care.
• Visit clients routinely in order to observe and evaluate each clients’ physical and emotional status.
• Maintain the confidentiality of all client care information.
• Monitor nursing care to ensure that all clients are treated fairly, and with kindness, dignity, and respect.

  Monday - Friday with a weekend rotation
• On Call:
  Yes, weekdays and a weekend rotation
• Holiday Rotation: 1 summer and 1 winter

• Required Skills
  • At least 1 year experience in Home Care preferred
  • 1-2 years RN experience in acute care
  • Valid RN License in the State of Michigan in good standing
  • Ability to work in a variety of settings, good communication skills
• Preferred Skills
  • Previous experience with Well Sky and Forcura
  • Ability to handle multiple tasks simultaneously and meet deadlines
  • Excellent organizational and time management skills
  • Self-starter
