Overview
Registered Nurse with expertise in stroke care to join our Neuroscience team as Stroke Coordinator, overseeing and advancing stroke program services across Southeast Michigan. The Nurse Stroke Program Coordinator provides coordination and oversight of stroke program services and quality across the Southeast Michigan (SEMI) Market. This position supports the implementation and sustainability of medical, service line, and interdepartmental components of the program.

Description: Registered Nurse RN – Stroke Program Coordinator Neuro
Responsibilities- Coordinate services to meet program goals and metrics while fostering a safe, collaborative environment.
- Assist with operations, quality of care, and program development; maintain protocols, policies, and procedures.
- Capture, analyze, and report data to support leadership in improving care and efficiency; conduct audits and recommend process improvements.
- Support accreditation, certification, and regulatory compliance, including CMS and stroke program requirements.
- Act as a resource and liaison for patients, families, staff, and community partners; promote safe practices and positive experiences.
- Act as a resource for stroke patients and families, ensuring timely communication and follow-up care.
- Track stroke/neurovascular patients and confirm post-discharge appointments.
- Collaborate with physicians, clinics, EMS, and internal teams for a coordinated approach.
- Establish and maintain workflow with other Stroke Coordinators; identify improvements and set task expectations.
- Monitor program effectiveness with internal departments and community partners.
- Support continuity of care, assist staff nurses with stroke protocols, and lead community education sessions.

- LICENSURE:
Current Michigan Nursing License - EDUCATION:
Baccalaureate of Science in Nursing (BSN) degree from an accredited school of nursing. Masters of Science in Nursing (MSN) degree from an accredited school of nursing, preferred, not required. - CERTIFICATION:
Specialty credentialing in the clinical nursing practice specialty area (such as SCRN or CNRN) preferred, not required.
