Overview

The Clinical Supervisor, Hospice in collaboration with the Manager – Clinical Operations, is responsible for the provision of high quality patient care for hospice clients in one or more Interdisciplinary Teams (IDT). The Clinical Supervisor has primary responsibility for planning, implementing, and evaluating nursing and other skilled clinical services through systematic assessment of caseload and optimum utilization of personnel for assigned IDT(s).

The Clinical Supervisor will provide leadership oversite to all clinical employees in the assigned IDT(s) and collaborates with Manager – Clinical Operations to ensure appropriate high quality clinical care for all patients serviced by the provider number.

Key Responsibilities
  • Performs documentation reviews of clinical staff regarding direct patient care in the IDT(s) they oversee and report results to the Manager
    - Clinical Operations
  • Completion of in-home annual competency and evaluations for IDT clinical staff
  • Facilitates case management process with staff members, physicians and referral sources
  • Maintains open lines of communication with all members of the continuum of care team
  • Conduct ride-along with all clinical staff to ensure patient needs and hospice practices are being met as business needs arise
  • Educating staff on processes, procedures and policies as needed
  • Assign and monitor patient care schedules to ensure continuity and quality regulations
  • Monitor to ensure coordination and follow-through is complete for all referrals to the hospice program
  • Assist in the coordination and assignment of all newly admitted hospice patients to the appropriate team and/or disciplines
  • Ensure communication of all patient admissions and changes in Plan of Care is appropriately relayed to each member of the Interdisciplinary Team and Primary Care Physician
  • Participating in routine visits and admissions of patients in times of high census or low staff availability
  • Selects and maintains a qualified, well-organized staff to provide care for the needs of the patients
  • Is accountable and supervises the IDT team i.e. RN's, LPN's, Medical Social Workers, Chaplin’s, and Hospice Aides
  • Available to work after-hours to provide support and/or conduct in-person visits
  • Coordinate with the hospice physician to facilitate and run the IDG meetings in the absence of and/or in support of the Manager
    - Clinical Operations.
Key Technical Skills Required
  • Excellent verbal and written communication skills
  • The ability to assess patient needs and formulate individualized patient care plans to meet those needs
  • Proficiency in personal computer use, including e-mail, clinical, word processing, and spreadsheet software
Qualifications
  • A person who is a registered nurse.
  • A current unencumbered State professional license
  • Must maintain a valid driver’s license and good driving record
  • 3 or more years of nursing experience
Education/Training Preferred
  • Two years supervisory experience in an in-patient hospital, home care agency, or residential facility
  • Experience with in Hospice
