Completes comprehensive assessment of patient situations utilizing Case Management documentation standards, review of medical record, collaboration with health care team and discussion with patient/family.

In collaboration with patient, family, and primary care provider, develops plan to address and manage issues which influence health care utilization including services for home as well as facilitates hospital services.

Plans interventions and adjusts activities according to unit/patient needs. In accordance with unit or departmental practice, proficient with the electronic health records for documentation, assessment.

Instructs patient and documents education, both individually and via didactic groups. Provides patient care. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patient.

Based upon clinical assessments and considering age-specific, developmental and cultural needs of the individual, makes recommendations to physicians regarding appropriate service needs and works collaboratively with the health care team.

The Social Worker/Clinical Therapist provides inpatient therapy focusing on the treatment of mental illness and/or chemical dependency. Demonstrates a working knowledge of mental health and substance use disorders.

Provides various social work services to meet needs of patients and family members. Provides various direct services including crisis intervention, counseling, discharge planning, coordination of services.
