Care is searching for a Weekend
RN, Registered Nurse to perform home health
visits with our North Boston Senior Living team.
This office delivers exceptional care to patients living in senior living communities (assisted living / independent living) throughout the North Shore and Merrimac Valley area.
Schedule:
a combination of weekend days that include Friday, Saturday, Sunday, or Monday.
Please note:
As this is mostly a Home Health SOC / Admission RN position, prior or current experience in Medicare-certified Home Health and OASIS documentation is required.
Qualifications:
- A current license as a Registered Nurse in Massachusetts
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home health / OASIS documentation experience strongly preferred.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
- Ability to travel to cases as assigned.

- Follow a designated care plan in accordance with patient's needs
- Make home visits to clients in designated geographic territories
- This position may also include seeing clients in the traditional home health community setting
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures
- Monitor clients' conditions; reporting changes to Clinical or Client Services Manager
- Follow up with, execute, and properly document doctors' orders
- Perform client assessments as necessary
- Case management and coordination
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided
- Pay: $58-63 per productivity point, based on experience and negotiable
- Mileage reimbursement
