• Provide compassionate, high-quality nursing care to home health patients
• Perform comprehensive patient assessments and develop individualized care plans
• Accurately document patient progress using OASIS and HCHB systems
• Collaborate with interdisciplinary healthcare teams to optimize patient outcomes
• Educate patients and families on health management, preventive care, and clinical support
• Maintain compliance with all regulatory and documentation requirements
