Physical Therapist Assistant, Home Health

Overview

Center Well Home Health is hiring a Physical Therapist Assistant, Home Health. This role supports patients in their homes with rehabilitation to regain strength, mobility and independence.

• Administer the physical therapy treatment plan as developed by the Physical Therapist.
• Administer non-complex active and passive manual therapeutic exercises, therapeutic massage, traction heat, light, cold, water, and electrical modalities to patients.
• Accurately document daily, weekly, or monthly records of clients’ progress and submit clinical paperwork on time.
• Attend educational meetings and contribute to department in-services.
• Notify the physical therapist or Executive Director of equipment failures, supply shortages, staff or client changes, and other departmental issues.
• Schedule clients and perform varied office tasks.
• Apply infection control and universal precautions in patient care activities.
• Accept clinical assignments consistent with education and competence.
• Promote compliance with fiscal intermediaries and/or third-party payors through education and coaching as necessary.

• Associate degree in Physical Therapy from an American Physical Therapy Association program.
• Current state license to practice Physical Therapy.
• Valid driver’s license, auto insurance and reliable transportation.
• Proof of current CPR certification.
• Minimum two years experience as a Physical Therapy Assistant.
• Comprehensive knowledge of changes in the geriatric population.
• Self-directed, enthusiastic, and accepts constructive feedback.
• Good verbal and written communication and organization skills.
• Professional image and attitude for patients, families, coworkers, and others.
• Meets mandatory continuing education requirements and maintains clinical competency.
• Ability to work a flexible schedule and travel locally.
• Able to maintain confidentiality.
• Must read, write, and speak fluent English.
• Good and regular attendance.

• Base pay range: $58,400 - $80,000 per year (40 hours/week).
• Pay per visit/unit: $37.00 - $52.00, per visit.
