Responsibilities
- Conduct patient assessments and develop individualized care plans.
- Coordinate discharge planning and transitions of care for acute care patients.
- Collaborate with physicians, nurses, and other healthcare professionals to optimize patient outcomes
- Provide patient and family education regarding treatment plans and care coordination.
- Document patient care accurately in Epic.
- Identify barriers to care and implement strategies to address them.
- Participate in quality improvement initiatives and case review processes.
- Acute Care nursing experience
- Proficiency in Epic electronic medical record system
- Strong clinical assessment and critical thinking skills
- Excellent communication and collaboration abilities
- BLS (required)
- AK RN License (required)
- ACM Certification (preferred)
Charting:
Required, Epic
Shift: Day 5x8-Hour (07:30 - 16:00)
Job Type: Contract
Pay: $75.20 per hour
Work Location: In person
