SCF is internationally recognized for its relationship‑based Nuka System of Care, which emphasizes shared responsibility, continuity, and whole‑person wellness.
Customer-owner: This term gives those receiving care to not only be customers, but also owners of their own health and owners of the care system by providing feedback that will guide future improvement. It signifies a substantial change in the relationship between customer-owners and care providers. More than just diagnosing illnesses and prescribing treatment, SCF’s providers work in relationship with customer-owners to help them achieve whole person wellness.
Relationships: When providers build strong relationships with customer-owners, it helps providers better understand the health issues, social factors, and family history that customer-owners are facing, and gives them the opportunity to help them make healthier choices in their lives. In addition to these relationships, SCF employees build strong relationships with each other and SCF maintains a strong relationship with the community.
