Responsibilities
  • Provide direct patient care and support under the supervision of registered nurses.
  • Administer medications and monitor patient responses to treatments.
  • Assist in developing and implementing patient care plans.
  • Participate in ongoing professional development and training opportunities.
  • Maintain accurate patient records and documentation.
  • Collaborate with healthcare team members to optimize patient outcomes.
  • Engage in performance measurement and strive to meet individual targets.
  • Monitor vital signs and report any changes to the nursing staff.
  • Educate patients and families on health management and care procedures.
  • Ensure compliance with healthcare regulations and standards.
  • Assist with daily living activities for patients as needed.
Experience/Qualifications
  • Valid LPN/LVN license and current CPR certification.
  • Commitment to ongoing professional development and training.
  • Experience in direct patient care in a clinical setting.
  • Ability to monitor vital signs and report changes accurately.
  • Willingness to engage in performance measurement and target achievement.
  • Strong communication skills for patient and team interactions.
  • Ability to work independently and within a team environment.
  • Understanding of healthcare regulations and compliance standards.
  • Compassionate approach to patient care and support.
  • Familiarity with electronic health records (EHR) systems.
