Summary

Under general supervision, provide basic patient care to all patients, including isolation patients. Deliver age‑specific assistance with activities of daily living and therapeutic measures that reduce agitation or increase comfort. Under direct supervision, ensure patient safety through observation. Level I employees are new graduates or newly certified within their first two years of practice;
Level II have at least two years of practice with advanced training, certification, and precepting experience;
Level III perform high‑complexity tasks, assist with process improvements, train lower‑level employees, and work with minimal supervision.

Responsibilities

Observe patients with suicidal ideation or behavioral health concerns; report significant changes to the primary RN. Record and report information within scope of practice. Conduct regular environmental assessments for safety. Provide basic care (hygiene, grooming, dressing, toileting, preventive skin care, oral hygiene) and assist nursing staff with mobility and safe transfer techniques. Obtain and record vital signs, temperature, height, weight, and other specified parameters;
