  • Formulate service plans by assessing households' needs and establishing short-term goals.
  • Regularly reassess households' needs and goals to ensure ongoing relevance.
  • Cultivate and manage a network of resources in collaboration with other community-based providers.
  • Identify and secure appropriate resources for households through research and adherence to best practices.
  • Facilitate referrals to relevant resources for households and ensure follow-up for successful linkages.
  • Ensure households possess adequate medical insurance and are connected to appropriate healthcare providers.
  • Collaborate with Housing Navigator and other departments at Illumination Foundation to support clients in transitioning to permanent housing, including assistance with move-ins.
  • Encourage and ensure client participation and engagement in Illumination Foundation activities.
  • Provide guidance to parents on parenting, child development, school attendance, and familial concerns, offering resources for children as needed.
  • Monitor children for signs of emotional or developmental issues, promptly communicating concerns to the supervisor.
  • Monitor and report signs of emotional, physical, and sexual abuse, complying with legal obligations to inform CPS when necessary.
  • Advocate for clients during appointments related to housing, health, education, and social and emotional well-being.
  • Provide transportation for client appointments when necessary.
  • Conduct regular home visits and wellness checks.
  • Ensure residency adheres to habitability standards.
  • Participate in individual supervision sessions to discuss the program, organization, client progress, and strategies for improvement.
  • Actively engage in case conferences and team meetings.
  • Maintain accurate and updated case notes, records, and relevant paperwork using KIPU and HMIS.
  • Establish and manage confidential case files for participants and prepare required statistical reports for program management and evaluation.
  • Demonstrate commitment to Illumination Foundation's mission and core values, fostering respect and harmony among colleagues.
  • Motivate staff, acquire new skills/procedures, and promote a positive attitude toward change.
  • Handle phone calls and emails professionally and promptly.
  • Exercise sound judgment and utilize best practices when working with clients.
  • Uphold ethical standards, reflecting integrity, transparency, accountability, respect, and responsibility in all actions on behalf of Illumination Foundation.
  • Model openness, honesty, and accountability to colleagues, volunteers, donors, and all others associated with Illumination Foundation.
  • Maintain confidentiality of work-related information and materials.
  • Accept personal responsibility and accountability for job performance.
  • Possess a comprehensive understanding of the needs of the low-income/homeless population.
  • Demonstrate passion and commitment to serving those experiencing homelessness.
  • Effectively utilize interpersonal and communication skills, including tact and diplomacy.
  • Collaborate with a multidisciplinary team, clients, and the general public.
  • Work independently, manage time effectively, prioritize tasks, and exercise good judgment with minimal supervision.

Qualifications:

  • 2 years of experience and/or Bachelor's degree in Social Services, Healthcare, or a related field.
  • Experience in homeless services, case management, mental health, or permanent supportive housing.
  • Valid CA driver's license.
  • At least 1 year of experience working with at-risk/homeless individuals (preferred).
  • Familiarity with VI-SPDAT.

Preferred:

  • Bilingual in English and Spanish.
  • Experience in non-profit housing and/or housing for people with disabilities and chronic health conditions.
  • Proficiency in Google Suite.
