Job Summary

We are a community-based adult day program focused on behavior management for adults with developmental disabilities. We provide vocational training, community integration, and personalized support to help our participants achieve their goals in a positive, engaging environment. This role is full-time, Monday to Friday, 8:30 AM–4:00 PM. All positions require dedication, compassion, and experience in adult day programs for behavior management. A $500 sign‑on bonus is offered for each position (conditions apply).

Key Responsibilities
  • Develop Individual Plans:
    Create and maintain individualized service plans (ISPs) or program plans for each participant, in collaboration with the participant, their family, and the team. Ensure plans align with each person’s IPP goals and behavioral support needs.
  • Service Coordination:
    Coordinate all services and supports for participants. Act as a liaison with families, residential caregivers, Regional Center coordinators, and other service providers to arrange needed supports (therapies, medical appointments, transportation, etc.).
  • Monitor Progress:
    Track participant progress towards goals. Collect input from direct support staff and behavior specialists on participant achievements and challenges. Regularly review and update plans to reflect progress or changing needs.
  • Documentation & Compliance:
    Maintain thorough case files and documentation for each participant, including progress notes, quarterly reports, and any incident reports. Ensure all documentation meets the standards and regulations for adult day programs (e.g., regional center and state requirements).
  • Team

    Collaboration:

    Work closely with counselors/direct support staff and the ABA Specialist to ensure that daily programming and behavior strategies are implemented according to each participant’s plan. Provide guidance and training to staff on individual participant needs and plans.
  • Advocacy:
    Advocate for participants’ needs and rights. This includes facilitating participant meetings (such as annual IPP meetings or interdisciplinary team meetings), voicing any concerns about participant welfare, and helping problem‑solve any barriers to success.
  • Quality Assurance:
    Ensure the health, safety, and well‑being of participants at all times. Participate in program evaluations and help implement improvements. Stay up‑to‑date with best practices and regulations in the field of developmental disability services.
  • Other Duties:
    Perform other case management and program support tasks as assigned by the Program Director to help the program run smoothly.
Qualifications & Requirements
  • Education:

    Bachelor’s degree in Social Work, Psychology, Human Services, or a related field required. Master’s degree or additional certifications in the field are a plus.
  • Experience:

    3–5 years of case management or related experience in the developmental disabilities field required. Experience should include working with adults in a day program or similar setting, with an understanding of behavior management principles.
  • Knowledge/

    Skills:

    Familiarity with Individual Program Plan (IPP) processes and regional center or similar systems is highly desirable. Excellent organizational skills and attention to detail for managing multiple cases and documentation. Strong communication skills (both written and verbal) for interacting with families and professionals. Empathy, problem‑solving abilities, and a collaborative mindset are a must.
  • Language:
    Fluent English required. Fluency in Arabic or French required (must be able to communicate in at least one of these languages).
  • References:
    Must provide professional references who can speak to your experience and reliability in a case management or similar role.
  • Background & Health:
    Must pass a criminal background check and have a clear TB test result. CPR/First Aid certification required (or willingness to obtain prior to hire).
  • Other Requirements:
