Position Details

Island Health is seeking an experienced Case Manager to join our dedicated Care Management team. In this key role, you’ll collaborate with providers, patients, families, and payors to coordinate care, guide safe discharge planning, and ensure appropriate utilization of hospital services. As a member of the interdisciplinary Care Management Team, the Case Manager - MSW will utilize a collective process to perform care coordination, discharge planning and utilization review to promote quality, cost effective outcomes for patient.

What You Will Be Doing
  • Discharge Planning
    :
    Identifies patients at risk for encountering problems post-hospitalization through performing a comprehensive assessment of patient discharge needs.
  • Works with patient and interdisciplinary care team members to coordinate and implement patient discharge planning.
  • Facilitate referrals to outside agencies to facilitate safe and appropriate discharge plan.
  • Provide in-depth psychosocial assessment and identifies appropriate interventions or referral to assist patient / families.
  • Provides in-depth chemical dependency, mental health assessments. Connect patient and/or family with appropriate resources and referral.
  • May provide guidance and serve as a consultant to the interdisciplinary care management team regarding complex psychosocial and/or discharge planning issues/barriers.
  • Adequate knowledge of community services, resources and facilities to support discharge planning.
  • Ability to prioritize and manage a daily patient caseload; includes ability to continually analyze and evaluate daily assigned caseload and prioritize the patients that require an initial or on-going discharge assessment, readmission risk assessment or discharge plan, which patients are discharging and need post-hospital placement and continued follow-up care needs.
  • Assure timely and appropriate documentation of discharge assessments and plans, utilization review and any necessary follow-up care to assure safe and cost efficient continuity of care.
  • May collaborate with community resources and be a liaison between Care Management and agencies as appropriate for the benefit of discharge planning.
  • Communicates patient discharge plan with appropriate stakeholders in a timely manner.
  • Documents clearly all assessments, interventions and planning in a clear, concise, accurate and timely manner.
  • May perform initial and concurrent medical record review for third party payers.
  • May review medical records for medical necessity of hospital admission and continued stay using Interqual or other assigned guidelines.
  • Educate and include patient and family in transition planning in preparation for discharge and provides updates and/or changes throughout the hospital stay.
  • Contact and arrange placements to skilled nursing facilities, rehab, long-term care, specialty care, home health agencies.
  • Other duties as assigned dependent upon training and scope of practice.
