A respected, high-performing hospice organization is seeking an exceptional Hospice Registered Nurse (RN) to step into a full-time Case Management role. This opportunity is being recruited confidentially for a select client known for clinical excellence, strong leadership support, and a patient-first culture.

This role is ideal for a seasoned hospice professional who values autonomy, professionalism, and delivering elevated end-of-life care.

The Opportunity

As a Hospice RN Case Manager, you will serve as the primary clinical leader for your assigned patients, guiding care with expertise, compassion, and precision. You will work closely with an interdisciplinary team in an organization that prioritizes quality outcomes, ethical practice, and clinician respect.

Responsibilities
  • Provide advanced hospice nursing care and oversight for a defined patient caseload
  • Conduct comprehensive assessments and ongoing clinical evaluations
  • Develop, implement, and manage individualized plans of care
  • Collaborate with physicians and interdisciplinary team members to ensure seamless care delivery
  • Serve as a trusted clinical resource and advocate for patients and families
  • Ensure regulatory compliance, accurate documentation, and adherence to hospice standards
  • Participate in on-call rotation as needed
Ideal Candidate Profile
  • Active Registered Nurse (RN) license in good standing
  • Minimum 2+ years of hospice nursing experience (case management strongly preferred)
  • Demonstrated expertise in end-of-life symptom management
  • Strong clinical judgment and exceptional organizational skills
  • Professional presence with excellent communication and leadership ability
  • Ability to work independently while thriving in a collaborative environment
