Summary

The LVN assigned in the in‑patient care areas demonstrates active collaboration and communication with RN, Patient, Primary Care Provider, and other care team members.

Qualifications
  • United States Citizenship:
    Non‑citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education
    • Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing (NLNAC) at the time the Mojon was completed.
    • Exceptions include military service health‑care education or training accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure.
    • VHA employees who were converted to appointments under 38 U.S.C. 7401(3) or 7405 are excluded from this waiver.
  • Licensure
    • Full‑active‑current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory, Commonwealth, or District of Columbia.
    • Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
    • May qualify under the Grandfathering Provision for current VHA employees who meet the criteria.
  • Grade Determinations
    • GS‑3: LPNs expected to perform all duties within the scope of their license (e.g., medication administration) and may require higher‑level review depending on the complexity of duties.
    • GS‑4:
      Employees may be appointed or advanced to GS‑4 based on qualifying experience or education criteria described in paragraphs 3b‑2c.
    • GS‑5:
      At least one year of qualifying experience at GS‑4; capable of providing comprehensive practical nursing care and instructing less experienced staff.
    • GS‑6:
      At least one year of qualifying experience at GS‑5; technically proficient in initiating, performing, and completing assigned duties; able to observe, identify, and respond to patient needs.
  • Basic Life Support (BLS) required.
  • Preferred Experience:

    One year of acute care or long‑term care experience.

Duties
  • VA Careers – Licensed Practical Nurse:
  • Perform daily care for residents including activities of daily living, dressing, hygiene care, nail care, toileting, restorative programs, assistance with feeding, decluttering, end‑of‑life care, and post‑mortem care.
  • Demonstrate a caring, compassionate, and respectful attitude.
  • Assume a peer education and mentorship role with lower‑level LVNs, and serve as a team leader under RN guidance.
  • Complete wound care treatment as ordered by the physician agregar documentation and assessment for preventative wound care treatment.
  • Maintain a working knowledge of each neighborhood on unit and assist other staff as necessary to ensure proper care.
  • Gather information about patient condition and communicate to RN to develop a specific plan of care.
  • Understand and use nurse call and Wander guard systems, responding promptly to alarms and patient situations.
  • Complete daily/shift documentation; not limited to CLC Wound Care & Treatment Note.
  • Use appropriate infection control measures as listed in the orientation check‑off packet.
  • Obtain intake and outputs accurately (oral intake, IV pump, feeding pump, Foley catheter, urinal, drains, etc.) and report shift totals/deviángates to RN.
  • Collaborate with team, committees, work groups, and projects related to Veteran Care implementation.
  • Assist provider and/or RN with data gathering, chart reviews, and documentation.
  • Prepare specialty clinic for provider and patients by ensuring supplies and equipment, lab/diagnostics ordering, and results availability.
