BLS
SSN Required
DOB Required
References: 2 References in the last 36 months

Associate degree from a CAPTE-accredited PTA program (Commission on Accreditation in Physical Therapy Education)
Certification from the National Physical Therapy Exam (NPTE) for PTAs, administered by the Federation of State Boards of Physical Therapy (FSBPT).
TX License

Resume
Must have candidates current name, no nicknames, or abbreviations. Work history must include name of facility worked, city and state, title, department, and dates employed from the last 7 years. Agency names are not accepted as the employer. Highest level of education listed, including month/year of graduation and degree achieved if applicable.

Skills Checklist
Completed within one (1) year of submittal for specialty where candidate is submitted

Two (2) References
Two (2) professional references from work history occurring within the past three (3) years. One must be from a supervisor (charge nurses are considered supervisors). Reference must include the full name (first & last name) of the person providing reference. The name of the facility and the dates of employment must match the resume. Reference must include a breakdown of skills and strengths with a rating score.

Letters of recommendation will not be accepted.
