Seniors at Home, one of the leading providers of innovative home health care, seeks a hands-on Home Health Aide for an entry-level position with growth potential. This role requires direct work with frail patients, so patience, compassion and flexibility are essential. The Home Health Aide plays a vital role in delivering compassionate, personalized care to our seniors. This position is based on-site at our Altoona location, where you’ll provide one-on-one support in our Day Room setting.

Key responsibilities include

• Providing direct, individualized care in a structured day program environment
• Assisting with personal care tasks such as bathing, grooming, dressing, and oral hygiene
• Supporting therapeutic exercises and activities as guided by on-site physical, occupational, and speech therapists
• Administering medications under the supervision of an on-site Registered Nurse (RN)
• Observing and reporting changes in patients’ conditions to RNs or therapists
• Promoting a safe and supportive environment, reporting any potential safety concerns
• Accurately documenting patient care and activity as required
• Performing additional duties based on individual care plans or team direction
